Registration and Payment 


Registration Process:
  1. Complete the online registration here: http://goo.gl/forms/bbb1U0X7qxzOMcP42
  2. Write a cheque for the balance owed and make it payable to "Patrick Suen"
  3. Send your cheque to Patrick Suen @ 1904-181 Wynford Drive, Toronto, ON M3C 0C6, Canada
  4. A waiver form will be provided during check-in on the day of the seminar
Deadlines:
The general registration deadline is July 9, 2016 and cheques must be received by July 13, 2016. 
Registrations and cheques will still be accepted after those respective deadlines, however, an administration fee of $30 CDN will be added to the total, and lunches will not be guaranteed.

Registration Changes and Cancellations:
Changes can be made by submitting a new registration in the form below. The latest submission will count as official. 
The last day to change or cancel your registration with a full refund is July 9, 2016.
Changes or Cancellations after that date will incur an administration fee of $30 CDN. 
Participants who submit NSF cheques will also be charged an administration fee of $30 CDN.

Forms of Payment:
Cheques will be the primary accepted method of payment. Cash will also be accepted in-person to Patrick Suen or Hanna Ikeda-Suen. We have decided not to use an online payments system to avoid processing fees and keep the cost as low as possible; as per feedback received from the pre-registration form.

Lecture/Interview:
Participants are invited to submit questions to be proposed for the Lectures (Saturday or Sunday) or the Interview (Banquet). Sensei will review prior to the sessions.
